You will have to adjust the health and mana to your needs.

The bot will cast spell 2 when out of form, which casts cat form.
The bot will cast spell 2 when in cat form to attack
The bot will cast spell 2 when in bear form to attack
The bot will cast bear form when it detects 2 targets
When the bot casts heal, it cancels form and heals, waits to use spell 2 which casts cat form – bot then uses spell #2 to attack in cat form
Adjust macro to your spells and needs
